To improve the communication process between patients/families and among healthcare providers. This process includes patients, families, physician, nurses, patient care techs, as well as ancillary departments providing care for the patient. Patient satisfaction survey showed low scores in our coordination of care category, showing a lack of effective communication during patient admissions. Previous communication process was ineffective between healthcare providers as well as between patients/family. Target population is CWW7 – 21 bed unit. The significance of this project is to utilize whiteboards as a communication tool to improve the communication process, team work and patient centered care. Guidelines or data templates were communicated to the staff and included the date, name of nurse & tech, current vital signs for shift, glucose monitoring, daily weights and I&O. Follow up performed by Nurse Manager and monthly reviews of patient satisfaction survey scores were performed.
