Managers, leadership (head nurses, advanced nurse clinicians, clinical resource nurses, etc.), and frontline staff recognized the need to incorporate effective Hand-off Communication on a consistent basis so that the process would become a hardwired practice hospital wide. The utilization of effective hand-off communication is supported by evidence based research and provides that opportunity to incorporate the critical component of patient and family involvement. The main objective was to have successful implementation of this process by allowing frontline staff to assist in the development and to have ownership of the process and project success. The target populations were the department managers, the leadership staff listed above, the frontline staff, and the patients along with their families.
