Transitions between healthcare settings and home have been identified as a major contributor to patient adverse effects and medication errors often leading to readmission to the hospital. Common reasons for readmission include avoidable adverse drug events and poor patient adherence to medication regimens. The presentation is a summary of a one year pilot, implementing a standardized process to evaluate maintenance therapy with emphasis upon streamlining the medication regimen. Results include a reduction in 90 day readmissions.
